A stay begins with an evaluation by our team of physicians, therapists, nurses, social services, and other support staff. Our team is available 24/7 to help you receive the highest level of care during your stay with us. Our goal is to help you be safe, healthy, and happy.

Diabetes Management
Our skilled nurses help with all parts of managing diabetes. These include:
- checking blood sugar
- blood glucose monitoring
- insulin administration and tracking
They also assist with exercise plans, meal planning, education, and more.

Restorative Nursing
At Oaks Post Acute, we aim to improve the quality of life for our long-term care patients that need special restorative programming. These plans focus on daily living activities such as:
- bowel and bladder training
- nutrition and weight monitoring

These activities also include improving movement, strength, and range of motion.

Complex Wound & Tube Management
Our wound care program utilizes professional, clinical, and technical resources designed for the treatment of a variety of wound conditions including:
- Ostomies (colostomy/urostomy)
- Wound dehiscence
- Diabetic arterial/venous ulcers
- Pressure ulcers
- Pre- and post-flap surgery

Pain Management
We understand that pain can delay and interrupt the progress of rehabilitation. We work hard to keep our patients comfortable. We use medication and other pain management methods. This helps patients take part in therapy and gain its benefits. We also give our patients helpful information and resources. These will help you manage temporary, long-term, or chronic pain at home.

Dietary Program
Our dietary program includes meals to meet specific dietary and therapeutic needs, according to physician orders. Our residents recieve regular nutritional assessments from our on-staff registered dietitian. We also offer oral supplemental diets, enternal feeding, and hydration programming.

Respite Care
Our respite program offers safety and supervision in a caring environment. This gives caregivers peace of mind. Our providers design respite care to meet the physical and medical needs of a resident during a short-term stay.

Hospice Care
